$25 Visit Fee (Primary Care Physician/Urgent Care) $50 Visit Fee (Specialist) for all eligible injury or illness outpatient office visits at a primary care physician, specialist or at an urgent care.

 

Membership shares up to a maximum of a $150 per office visit for eligible medical services. 

 

MAXIMUM OFFICE VISITS PER MEMBERSHIP YEAR of 2 or 3 visits, per member, based on the Aspire 250-500-1000 you have selected.

Aspire

  1. There is 12-month pre-existing condition limitation for all inpatient hospital and emergency room features.

  2. The hospital admission features for defined condition has a 30-day waiting period. No features are eligible for any defined condition where the date of diagnosis is during 30-day waiting period.

  3. The daily hospital admission features is eligible for either heart attack or heart disease during a membership year, but not both.

  4. Daily features for eligible surgery are scheduled and vary based on the specific surgical procedure performed.

  5. Daily features for eligible anesthesia vary and are equal to 20% of the applicable surgery feature.

Aspire 2

  1. Network Provider: PHCS -The Aspire 250-500-1000 provides these preventative services only through a PHCS provider.

  2. Office Visit Fees: Your doctor may provide a preventative service, such as a cholesterol screening test, as part of an office visit. Please note, that you may have to pay some costs of the office visit, if the preventative service is not the primary purpose of the visit, or if your doctor bills you for preventative services separately from the office visit.

  3. Talk to your health care provider to know which eligible preventative services are right for you based on your age, gender, and health status.
